The intake form is deliberately longer than a contact blurb because context changes the quality of what we prepare. Tell us how time moves through your household, what you want to practice, and how you like to receive follow-ups. Nothing here replaces medical advice; it simply helps us meet you where you are.

We read every field before suggesting a first conversation. Your schedule snapshot helps us avoid proposing prep blocks that collide with school pickup or night shifts.
